1. Form + NOC
2. ID Copy of Sponsor (or ID Copy of Authorized Person for Sing in Computer Card)
3. ID Copy Employee (Requester for Profession Change)
4. Educational Certificate (Translated in Arabic and Attested by Embassy)  
5. Computer Copy
6. Old Agreement Copy
7. 3 Set Newt Agreement Copy (Signed and Stamped by the Company and Employee) 
8. Valid Municipality License (Rukhsa) Copy
9. C R (Commercial Registration) Copy
10. Electricity Bill Copy of Company
11. Salary Transfer Bank Statement for Six Months

if you haven't old agreement copy, you have to make a agreement online with old profession, if you want to know "How to Make Agreement Online" then Click following link 

also you can learn how to fill the form, NOC and how to make a New Agreement with new profession, i am attaching form, NOC and New Agreement Simple, you can not make new agreement online, to make new agreement need manually typing,
